If you do not provide tracking details, Cochon555 is not responsible for any packages not making it to the event. We hope all packages are on-site Thursday before the event, we check packages then and will let you know if they are not on-site, if they are, you will not hear from us. All tracking numbers due on Monday, along with your guest list below.

Providing guest names allows them easy access to the event. If we do not get your your names by Monday for each event, you tickets will simply be listed under your COMPANY NAME. We close the guest list on Tuesday evening.

Per our agreement, Cochon555 will provide agreed upon equipment, staffing, and ingredients. Any special requests needs have to be addressed on this form.
PLEASE READ OUR COMPLIANCE / ALCOHOL POLICY. *
At the end of each event the COCHON555 staff will collect all remaining (open and unopened) alcohol. To ensure the safety of all venders and guests we need your support in explaining this policy to your day-of representatives instructing them to surrender left-overs at the end of the event. In the past, vendors gifting alcohol has created dangerous situations, leaving the event, its' partners and host, and the cause at large assuming an unnecessary liability. We appreciate your support in helping us preserve the celebratory nature of each event.
Names will be placed at Will Call. Simply state YES if you added extra names above. You will be Invoiced for tickets with 20% discount.

ALCOHOL COMPLIANCE - #1 - Never SELL or GIFT any bottle of alcohol to any guest of the event - #2 - Allow Cochon representatives to collect product before event for judges room and chef gifts - #3 - On-Site representatives will read and initial a copy of Alcohol Policy upon arrival at events.
